Is it low testosterone, or something else?

Fatigue, low libido, brain fog, and low motivation are the classic complaints men bring me. They are also symptoms of sleep apnea, thyroid dysfunction, iron deficiency, insulin resistance, nutrient deficiencies, chronic stress, and some common medications. Treating a number on a lab report without finding out why it's off is how men end up on therapy that doesn't fix how they feel.

My training is in rheumatology, functional medicine, and longevity medicine, built on a chemistry degree, so I'm used to connecting dots across body systems. That's the difference you'll feel in the visit.

What I look at besides testosterone

Sleep & recovery

Poor sleep and untreated sleep apnea can lower testosterone and mimic its symptoms.

Thyroid

An underactive thyroid can look exactly like low T: tired, foggy, low drive.

Iron & nutrients

Low iron, vitamin D, B12, and other deficiencies quietly drain energy and mood.

Metabolic health

Insulin resistance and excess belly fat both lower testosterone and are worth addressing directly.

Medications

Some prescriptions affect hormones, mood, and libido. We review what you take.

Chronic stress and inflammation can suppress the hormone signals that drive testosterone.

Where TRT fits

When your symptoms and labs together point to low testosterone, TRT can be an effective part of your plan. Sometimes it is the right first step. Sometimes fixing sleep, thyroid, or metabolic health comes first, and sometimes it means you don't need TRT at all. I'll tell you which one it is.

Testosterone therapy can affect fertility. If you may want children, tell me at the start so we can plan around it. TRT is a prescription controlled substance and isn't right for everyone, and individual results vary.

Frequently asked questions

Do I have to start testosterone if my levels are low?

No. Low testosterone is sometimes the cause of your symptoms and sometimes a result of something else, such as poor sleep, sleep apnea, weight gain, insulin resistance, thyroid problems, or medications. We look at all of it first. TRT is recommended only when your symptoms and labs support it, and it is always your decision.

What symptoms of low testosterone do men notice?

Common ones are persistent fatigue, low libido, low motivation or drive, brain fog, mood changes, loss of muscle, increased belly fat, and poor recovery from exercise. These symptoms overlap with many other conditions, which is why a full evaluation matters before treatment.

What does TRT cost at Vitality?

The initial 90-minute consult with baseline labs is $150. Ongoing TRT management is $125 per month with automatic refills, and includes medication, injection supplies, and lab monitoring twice a year. We are a direct-care, cash-pay practice and do not bill insurance.

How is TRT monitored?

Treatment is monitored with follow-up labs, typically including testosterone, estradiol, complete blood count and hematocrit, and PSA when appropriate, along with blood pressure and a review of how you feel. Doses are adjusted to you, not to a number on a chart.

Will testosterone affect my fertility?

It can. Testosterone therapy can lower sperm production and may impair fertility. If you may want children, tell me up front; there are options to discuss before starting, and in some cases TRT is not the right first step.

Is testosterone therapy safe?
